Establish a framework to increase walking and biking trails and improve connectivity of non-auto paths and trails in a particular area

Expand broadband availability and adoption in unserved and underserved areas through initiatives that address infrastructure and cost barriers

Provide funding for local community development activities such as affordable housing, anti-poverty programs, and infrastructure development

Support financial institutions providing services to underserved and disadvantaged communities

Purchase the land a home is on to lease to homeowners with low and middle incomes and require homeowners to sell the home back to the trust or to another resident with low income upon moving

Support HPV vaccination through multi-component community interventions, including education, outreach, navigation support, and vaccine provision

Involve consumers in health care governance via roles on governing boards, advisory committees, or shorter-term special projects

Establish high deductible health plans paired with pre-tax medical expense accounts such as Health Reimbursement Arrangements (HRAs) or Health Savings Accounts (HSAs) and information tools

Provide education on basic budgeting, bank use, credit management, bankruptcy, credit building and counseling, homeownership, retirement, divorce, etc.

Support self-contained, homelike dwellings for 10-12 elderly adults who require nursing care; universal caregivers, usually CNAs, provide care and other supports while clinical teams visit for specialized care

Increase patients’ health-related knowledge via efforts to simplify health education materials, improve patient-provider communication, and increase overall literacy

Provide grants to states and localities to fund activities that build, buy, or rehabilitate affordable housing for rent or homeownership, or provide direct rental assistance to households with low incomes

Clean, remove, replace, or cover lead contaminated soil with non-contaminated soil, mulch, sod, grass, or concrete

Replace lead plumbing material such as pipes, service lines, fittings, solder, flux, and fixtures with non-lead plumbing material

Increase access to LARCs through cost reduction, comprehensive birth control counseling, provider training, efforts to ensure availability at local clinics, etc.

Provide funds to households with low incomes to meet home energy needs, especially for households with children, older adults, or individuals with disabilities

Provide funding via tax credits at the state and local level to support development and rehabilitation costs of low income rental housing

Integrate legal services into health care settings to address legal issues that affect health (e.g., housing, food, utilities); services provided by private practice lawyers, law students, etc.

Improve work environments for nurses via establishment of strong nursing leadership, organizational support, etc.

Establish roads that avoid built-up areas such as towns, cities, or commercial/business districts

Require employers in an affected jurisdiction to provide paid time off for employees to use when ill or injured

Make pricing for hospital procedures and other health care services publicly available, often via websites, online databases, report cards, or similar tools

Establish programs that accept expired, unwanted, or unused medicines from designated users and dispose of them responsibly

Make clinician, hospital, clinic, long-term care facility, and insurance plan performance on health care quality measures publicly available via report cards, reporting websites, or similar tools

Make health care facilities’ health care-associated or hospital-acquired infection (HAI) rates readily available to patients and providers

Prevent radon from entering occupied buildings and reduce existing indoor air radon levels via soil depressurization, home or room pressurization, heat recovery ventilation, etc.

Transition families and individuals experiencing homelessness into permanent housing quickly, often with supports such as short-term financial assistance, case management, landlord negotiations, etc.

Establish transportation services for areas with low population densities using publicly funded buses and vans on a set schedule, dial-a-ride transit, volunteer ridesharing, etc.

Provide reproductive health care services such as counseling, contraception, and testing in middle and high school-based health centers

Use counseling, informational materials, etc. to inform smokers and non-smokers of the harms of secondhand smoke and encourage them to implement home smoking bans

Provide permanent, basic rental housing with social services available on-site or by referral, usually for families with low incomes, older adults, veterans, or people with disabilities

Establish state level regulations that require nursing homes to employ at least a set number of licensed and non-licensed nursing staff, often set in terms of staff hours per resident day

Create designated tax districts that generate revenue to invest in affordable housing initiatives, blight remediation, and economic development efforts

Support school-, community-, and clinic-based teen pregnancy prevention programs such as comprehensive sex education, HIV/STI prevention and youth development efforts, service learning, etc.

Set the number, type, proximity, and density of tobacco retailers, especially near homes and schools, via state or local zoning, licensing restrictions, or other regulations

Shift the way health care organizations approach trauma by adopting universal trauma precautions and providing trauma-specific care

Use the purchasing power of employers and groups of insured individuals to create incentives and disincentives for health care providers to deliver high quality, high value care
