Use individual-, group-, and community-level interventions to provide education, support, and training that can affect social norms about HIV and other STIs

Address the needs of teenage mothers via clinic-based programs that provide health care and family planning services as well as case management, counseling, and other supports

Provide condoms free of charge or at a reduced cost in community and school-based settings

Provide tailored health information and assistance with decision making, behavior change, and emotional support via digital interactive programs

Provide teen mothers with services such as remedial education, vocational training, case management, health care, child care, and transportation assistance to support high school completion

Provide at-risk expectant parents and families with young children with information, support, and training regarding child health, development, and care from prenatal stages through early childhood via trained home visitors

Provide child care, parent education, physical health and mental health services, and other family supports to pregnant women and parents with low incomes and children aged 0 to 3

Arrange full-time foster care by relatives or adults who are not a child’s parent but have a family relationship with the child when a child is removed from home due to a safety concern

Provide a fixed, multi-component set of fall prevention interventions to older adults, usually in community settings, without an individualized risk assessment

Use an intensive, family- and community-based intervention that addresses individual, family and environmental risk factors that affect antisocial behaviors among serious juvenile offenders

Support the efforts of neighborhood residents to work together in preventing crime by reporting suspicious or potentially criminal behavior to local law enforcement

Provide health care services on school premises to attending elementary, middle, and high school students; physical and mental health services provided by teams of nurses, nurse practitioners, and physicians

Reduce patients’ out-of-pocket costs for tobacco cessation therapies such as nicotine replacement therapy (NRT) and cessation counseling participation

Place severely and chronically delinquent youth in foster families that are trained in structured behavior management and connected to TFCO program staff; formerly called Multidimensional Treatment Foster Care
