Work to partner with victims/survivors of intimate partner violence, help them with safety plans, and link them to community services (e.g., legal, housing, financial advice, emergency shelter, etc.)

Use a case management model for frail elderly patients living independently, coordinating aspects of long-term care (LTC) such as status assessment, monitoring, advocacy, care planning, etc.

Support locally-based visual, media, and performing arts initiatives for children and adults; also called participatory arts programs

Provide space to promote socializing among community members and offer programs and services such as recreational or educational activities, counseling or support services

Expand court-referred intimate partner violence offenders’ understanding of abuse, teach alternative reactions, and work to change gender role attitudes; also called batterer intervention programs (BIPs)

Establish a non-share capital cooperative model in which fee-paying members can share the communal resources of a grocery, house, or utility cooperative

Support a collaborative approach by a multidisciplinary team of professionals working to meet the full range of long-term care (LTC) needs for frail elderly patients living in community settings

Return the rights to land, property, and resources to Native people as part of policies and initiatives that promote tribal sovereignty

Provide continuous, comprehensive, whole person primary care that uses a coordinated team of medical providers across the health care system

Integrate legal services into health care settings to address legal issues that affect health (e.g., housing, food, utilities); services provided by private practice lawyers, law students, etc.
