New Hampshire Environmental Public Health Tracking Program This site provides data on health and environmental hazards and exposures, including the 2019 Community Health Outlooks. These individual community snapshots summarize 35 health-related indicators for 199 communities across New Hampshire. The chosen indicators represent community characteristics, risk factors, health behaviors, and health outcomes, thus reflecting a suite of conditions that impact health.


New Hampshire Health and Human Services Data Portal Health Statistics & Data Management provides access to the Behavioral Risk Factor Surveillance System and Youth Tobacco Survey data as well as data on various health indicators, including chronic diseases, communicable diseases, obesity, and health care access.
