Help participants share transportation via informal arrangements between individuals, formally arranged ridesharing programs, or other ride-matching services

Use a case management model for frail elderly patients living independently, coordinating aspects of long-term care (LTC) such as status assessment, monitoring, advocacy, care planning, etc.

Support locally-based visual, media, and performing arts initiatives for children and adults; also called participatory arts programs

Provide space to promote socializing among community members and offer programs and services such as recreational or educational activities, counseling or support services

Enhance streetscapes with greater sidewalk coverage and walkway connectivity, street crossing safety features, traffic calming measures, and other design elements

Establish high deductible health plans paired with pre-tax medical expense accounts such as Health Reimbursement Arrangements (HRAs) or Health Savings Accounts (HSAs) and information tools

Tailor health care to patients’ norms, beliefs, and values, as well as their language and literacy skills

Train volunteers, professionals, or paraprofessionals to help residents assess and reduce environmental home health risks and recommend low cost changes (e.g., improved ventilation, integrated pest management, etc.)

Provide grants to states and localities to fund activities that build, buy, or rehabilitate affordable housing for rent or homeownership, or provide direct rental assistance to households with low incomes

Provide rapid access to permanent housing and support (e.g., crisis intervention, needs assessment, case management), usually for chronically homeless individuals with persistent mental illness or substance abuse issues
