Roadmaps to Health Action Awards Announced  

Roadmaps to Health Action Awards have been given to 20 diverse communities throughout the nation. The Action Awards, which are designed to advance a community’s potential to improve health, are part of the County Health Rankings & Roadmaps (CHR&R) program.

Award communities will receive a year of direct, customized support from a Roadmaps to Health community coach and be able to participate in learning network activities. The Action Award communities will also receive $10,000 to accelerate their efforts in building a Culture of Health.

“The Roadmaps to Health Action Award communities are well-positioned to leverage the networking opportunities, coaching and funding this award offers to build on their existing momentum,” said Julie Willems Van Dijk, co-director of the County Health Rankings & Roadmaps. “These communities are committed to better health. We hope the next year of hands-on, coordinated support and learning enables these communities to take their work to the next level.”

The Roadmaps to Health Action Award communities include:

Bithlo, Florida    DuPage County, Illinois
Flint, Michigan Franklin County, Massachusetts
Jefferson County, Alabama Joplin, Missouri
Kansas City, Kansas La Crosse County, Wisconsin
Linn County, Iowa Manchester, New Hampshire
Monadnock Region, New Hampshire Petersburg, Virginia
Rancho Cucamonga, California Schenectady, New York
Shenandoah, Pennsylvania Springfield, Missouri
Stamford, Connecticut Swanton, Vermont
Vicksburg, Mississippi Yakima County, Washington
